PLAYER SAFETY & RESPONSIBILITY

Participation agreements

Review the terms every parent or legal guardian must accept before a player participates with Carolina Kings.

PRE-LAUNCH DRAFTEffective upon public launch · Last revised September 8, 2026

These terms are designed for Carolina Kings operations and should be reviewed by the organization’s insurer or attorney before public launch.

01

Liability waiver and assumption of risk

Basketball, conditioning, practices, games, tournaments, travel, and related activities involve risks including falls, collisions, illness, equipment failure, and serious injury. The parent or legal guardian knowingly permits the player to participate, accepts these inherent risks, confirms the player will follow safety instructions, and agrees to promptly disclose any condition that may affect safe participation. This waiver does not excuse conduct that cannot legally be waived.

02

Medical-treatment authorization

If a parent or guardian cannot be reached during an urgent situation, Carolina Kings personnel may seek reasonable emergency evaluation, first aid, ambulance transport, and treatment for the player. The family remains responsible for medical expenses and must provide accurate allergy, medication, condition, physician, and insurance information. This authorization does not replace the family’s responsibility to obtain required medical clearance.

03

Parent and player code of conduct

Players and families will demonstrate sportsmanship, respectful communication, punctuality, attendance, academic and personal accountability, care for facilities and equipment, and compliance with coach and event rules. Bullying, harassment, threats, fighting, abusive language, discriminatory behavior, sideline interference, and unsafe conduct are prohibited. Concerns should be raised privately through team leadership. Serious or repeated violations may result in discipline or removal without overriding the payment and refund policy.

04

Transportation permission

The parent or guardian is primarily responsible for transportation. When separately arranged or approved, the player may ride with an authorized coach, volunteer, team family, charter service, or other designated transportation. Seat belts and applicable safety restraints are required. Families must communicate transportation changes and emergency contacts. Acceptance does not obligate Carolina Kings to provide transportation for any event.

05

Photo and video release

The registration form allows the guardian to approve public/team use, limit use to private team communications, or decline identifiable use. Public/team permission may include photographs, video, livestreams, team graphics, website, social media, fundraising, recruiting, and program promotion without compensation. A guardian may request a prospective change by contacting the team, but materials already published or shared may not be fully retractable.

06

Payment and refund agreement

The family must review the registration information and included items before submitting payment. Personalized merchandise and costs already committed for a player may be non-refundable. Review the Payment & Refund Policy for withdrawal and refund terms.

07

Electronic-signature acknowledgment

Typing the parent or guardian’s full legal name, checking the electronic-signature consent, and submitting the form records the signer’s intent to sign these agreements electronically. The submission date, agreement selections, and registration details may be retained as the participation record. A family may request a copy or correction by contacting Carolina Kings.
